Hypothyroidism in Dogs

Hypothyroidism is a condition in which the thyroid gland produces less thyroid hormone than the body needs. Total T4

Purpose: Measures the total concentration of thyroxine (T4) in the blood, the primary hormone secreted by the thyroid gland. A normal or elevated result effectively rules out hypothyroidism, whilst a low result prompts further investigation.
Considerations: Total T4 can be suppressed by non-thyroidal illness, certain medications (including glucocorticoids and some anticonvulsants), and other hormonal conditions, leading to falsely low values in dogs that are not truly hypothyroid. A low value alone is not diagnostic and requires interpretation in the context of clinical signs and other test results.

Free T4 by equilibrium dialysis

Purpose: Measures the unbound, biologically active fraction of T4, which is less affected by binding protein fluctuations than total T4. This test can help distinguish true hypothyroidism from suppressed total T4 due to non-thyroidal factors.

Canine thyroid-stimulating hormone (cTSH)

Purpose: Measures the pituitary hormone that stimulates thyroid hormone production. In primary hypothyroidism, TSH levels typically rise as the pituitary attempts to compensate for low thyroid hormone output.
Considerations: Approximately 25 to 40 per cent of dogs with confirmed hypothyroidism have TSH levels within the reference range, limiting the sensitivity of this test. Thyroglobulin autoantibody test
