Circling or pacing

The dog or cat repeatedly walks in tight circles, often in one direction, or paces back and forth across a room without settling. It may appear restless, unable to find a comfortable resting position, or seem to be searching for something without a clear goal.

Vestibular Dysfunction

Disruption to the balance system, whether from inner ear disease or a problem further along the vestibular pathway, causes many animals to circle consistently toward one side, often together with a head tilt, unsteady gait, or abnormal eye movements. Onset is frequently sudden.

Middle or Inner Ear Disease

Infection or inflammation deep within the ear can affect the structures responsible for balance, producing circling that may initially look identical to primary vestibular disease, particularly when there is a history of chronic outer ear problems.

Cognitive Decline

Canine and feline cognitive dysfunction frequently produces circling or aimless pacing as part of a broader pattern of spatial disorientation, often building gradually and worse in the evening or at night.

Compulsive Behaviour

Repetitive circling or pacing can develop as a compulsive behaviour, sometimes triggered initially by stress, confinement, or an underlying discomfort, and then persisting as a repeated pattern even once the original trigger has passed.

Anxiety-Related Restlessness

Underlying anxiety can produce pacing as a displacement behaviour, particularly around triggers such as separation, unfamiliar visitors, or environmental changes, and tends to resolve once the triggering context passes.

Intracranial Disease

A structural change within the brain, including a mass, can produce persistent circling, typically toward the side of the lesion, often alongside other neurological signs that build over time.

Metabolic or Toxic Encephalopathy

Disturbances affecting brain function from a cause outside the brain itself, such as a congenital liver shunt or exposure to a toxin, can also produce disorientation and circling, sometimes worse after eating or alongside other systemic signs. This pattern is particularly recognised in young animals.

Why timing matters

Early observation

A sudden onset of circling, especially with unsteadiness or a head tilt, typically points toward an acute event affecting balance or ear structures rather than a slowly developing process.

Later presentation

Circling or pacing that has built gradually over weeks or months more often reflects a chronic process, such as cognitive decline, a slowly enlarging structural change, or an established behavioural pattern.

The pace at which the behaviour developed, together with whether it is stable, worsening, or fluctuating, helps distinguish an acute balance disturbance, which often improves over days to weeks, from a progressive or entrenched pattern that is unlikely to resolve without characterisation.

Conditions commonly associated

Vestibular Disease

Disruption to the balance system commonly produces circling toward one side, often with a head tilt and unsteady gait.

Otitis Interna

Inner ear infection can affect the vestibular structures directly, producing circling that closely resembles primary vestibular disease.

Otitis Media

Middle ear disease can extend to affect balance-related structures, contributing to circling alongside more typical ear signs.

Canine Cognitive Dysfunction

Spatial disorientation is a core feature of cognitive decline, and circling or aimless pacing is one of its more recognisable expressions.

Compulsive Behaviour in Dogs

Repetitive circling can develop into a self-sustaining compulsive pattern, sometimes originating from an earlier stressor or discomfort.

Anxiety Disorders in Cats

Underlying anxiety can manifest as pacing or circling, particularly around identifiable triggers or changes in the household.

Brain Tumours

A structural change within the brain can produce persistent circling, typically toward the affected side, as part of a progressive neurological picture.

Generalised Anxiety in Dogs

Generalised anxiety keeps an animal in a persistent state of heightened arousal, and repetitive movements such as circling or pacing act as an outlet for this unresolved tension, particularly when the animal cannot settle or identify a specific source of unease.

Portosystemic Shunt

A portosystemic shunt allows toxins normally cleared by the liver, such as ammonia, to bypass filtration and build up in the bloodstream, and once these reach the brain they disrupt normal neurological function, which can show as disorientation, aimless circling or pacing.

When to explore further

Circling that appears suddenly and consistently favours one direction, particularly alongside a head tilt or loss of balance, points toward the vestibular system rather than a behavioural origin.

Pacing that intensifies in the evening, alongside disrupted sleep or apparent disorientation in familiar surroundings, is a pattern more typical of cognitive change in an older animal.

Circling that is new in an animal with a long-standing history of ear discharge, head shaking, or recurrent ear infections may reflect extension of disease deeper into the ear.

Pacing or circling that appears tied to specific triggers, such as being left alone or during fireworks and thunderstorms, points toward an anxiety-related pattern rather than a primary neurological one.

Persistent circling that does not fit a recognisable trigger and continues to build over weeks, particularly with any change in appetite, coordination, or behaviour, may benefit from characterisation rather than observation alone.

Recording which direction the animal circles toward, how many episodes occur per day, and what time of day they cluster around builds a useful pattern over a short period. A video capturing the gait, head position, and any loss of balance during an episode adds detail that is easily missed when only recalled afterward, and noting any recent change in hearing, appetite, or sleep alongside the circling helps place it in a broader context.
