Sudden bulging eye

The eye appears to protrude further forward than normal, sometimes with the eyelids unable to close fully over it, and owners often notice this immediately after a fall, a fight, or firm restraint around the head or neck.

Traumatic Forward Displacement

A blow to the head, a dog fight, or even firm restraint by the collar or scruff can push the globe forward out of the orbit, particularly in brachycephalic breeds such as Pugs, Pekingese and Shih Tzus, whose shallow eye sockets and prominent eyes offer less structural protection than in longer-nosed breeds. This mechanism accounts for the classic scenario of a bulging eye appearing during or immediately after a physical altercation or a bout of vigorous restraint.

Retrobulbar Infection Or Abscess

Infection tracking back from an upper tooth root, most often a large carnassial or canine tooth, can build up behind the eye and push it forward over a day or two. Affected animals often show a preference for softer food or a reluctance to chew on the affected side in the days beforehand.

Retrobulbar Mass

A tumour or other space-occupying growth behind the eye, including retrobulbar lymphoma in cats, can gradually displace the globe forward. Because growth is typically slow, the change is sometimes first noticed only once it crosses a visible threshold rather than at its true onset.

Gradual Pressure-Driven Enlargement

Chronically raised pressure within the eye itself (glaucoma) causes the globe to enlarge and appear more prominent over weeks rather than minutes. This is a slower process that can still read as 'suddenly noticed' if the owner has not been looking closely, and it is mechanistically distinct from a globe that has been acutely displaced out of the socket.

Periorbital Swelling Rather Than True Displacement

An insect sting, contact allergen, or localised inflammatory reaction around the eye can cause the surrounding tissue and conjunctiva to swell, making the eye look as though it is bulging forward when the globe itself is still correctly seated. This is usually self-limiting and settles as the swelling reduces over a day or two.

Why timing matters

Early observation

In the short window immediately after a true traumatic displacement, the muscles and blood supply surrounding the globe are stretched but often not yet irreversibly damaged, and the shorter that window, the better the odds of the eye retaining both its structure and some visual function. Assessment of pupil response and the eye's general appearance in this early window can still be informative before secondary changes set in.

Later presentation

As exposure continues, the corneal surface, no longer covered by the eyelids, begins to dry and can ulcerate, while the stretched optic nerve and blood vessels are at increasing risk of permanent compromise. Once the cornea has dried and ulcerated, or if the surrounding muscles have been torn rather than simply stretched, the range of possible outcomes for the eye narrows considerably.

Brachycephalic breeds have a narrower safety margin than longer-nosed breeds because their orbits are shallower and the muscles retracting the eye are already comparatively short. A comparable degree of trauma or restraint therefore tends to progress faster and require less force to displace the globe in a Pug or Shih Tzu than in a longer-muzzled dog.

When to explore further

The eyelids can no longer be brought together over the surface of the eye, or the third eyelid is visibly drawn across a widened gap

The change follows a specific recent event, such as a fight, a fall, or restraint by the collar or scruff

There is reluctance to open the mouth, or visible discomfort along the jaw or upper cheek, pointing towards a dental origin

The corneal surface looks dry, dulled or increasingly cloudy compared with the other eye

The change has built gradually over days to weeks rather than appearing over minutes, which points towards a different underlying process than an acute displacement

A side-by-side photograph of both eyes from directly in front, taken as soon as the change is noticed and again shortly after, documents whether the difference is increasing, static or resolving. Noting the exact time the change was first seen, and whether it followed a specific event such as a fight, a fall or a walk where the lead was pulled taut against the collar, gives useful context. Watching whether the eyelids can still meet over the surface, and whether the animal is holding the eye partially closed, are both worth recording.
